How to Fill Out the South Korean Visa Application Form?
1. Personal Details

- Write your Last Name, then your First Name.
- If you have a Chinese name, write it; if not, put N/A.
- Check Male or Female.
- Write your birthday as Year/Month/Day.
- Tell your nationality.
- Write the country where you were born.
- If you have a National Identity Number, write it; if not, put N/A.
- Check ‘Yes’ if you’ve used other names for Korea, and provide details. Check ‘No’ if not, write N/A.
- Check ‘Yes’ if you’re a citizen of another country and specify it. Check ‘No’ if not, write N/A.
Photo Guidelines
- Photo must be taken within 6 months.
- Use a white background.
- Don’t wear a hat.
- Keep a neutral face.
- Paste the photo on the form.

2. Passport Information

- Tick your passport type.
- Write your Passport Number.
- Say where your passport was issued.
- Specify the location or office.
- Put the date it was issued.
- Enter the expiry date; make sure it’s valid for at least 6 more months.
- If you only have one passport, check ‘No’ and leave details empty. If you have more, check ‘Yes’ and fill in the Passport Type, Number, Country, and Expiry Date.
3. Contact Information

- Write your home address.
- Only fill this if it’s different from your home address.
- Put your cellphone number.
- Add your telephone number.
- Note your active email.
- Share emergency contact details: Full Name, Country of Residence, Telephone Number, and your relationship.
4. Marital Status and Family Details

- Pick your marital status.
- If you’re ‘Divorced’ or ‘Single,’ skip this part. If ‘Married,’ tell about your spouse: Family Name, Given Name, Date of Birth, Nationality, Address, and Contact Number.
- Say ‘No’ if you have no kids yet. Say ‘Yes’ and indicate the number of kids if you have any.
5. Education

- Indicate your highest education level. If it’s ‘Other,’ give details.
- Write your school’s name.
- Share where your school is located.
6. Employment Details

- Indicate if you’re working now.
- Share where you work, your job title, work address, and phone number.
7. Visit Details

- Pick why you’re going to South Korea.
- Say how long you’ll stay.
- Note your entry date within three months from applying.
- Give an address in South Korea; no need for reservations.
- Share the phone number you’ll use there.
- If you’ve been to South Korea in 5 years, say ‘Yes’ and give details; if not, say ‘No.’

- If you traveled outside your home country, say ‘Yes’ and share details; otherwise, say ‘No.’
- If you’re with family, say ‘Yes,’ give details; if not, say ‘No’ and write N/A.
8. Sponsor Info

If someone in South Korea is sponsoring you, check ‘Yes’ and give their details. If not, check ‘No.’
9. Trip Funds

- Estimate your total trip cost; ensure your bank shows enough funds.
- Indicate who’s funding you, with details: Name, Relationship, Support Type, and Contact Number.
10. Form Help

If someone assisted, check ‘Yes’ and give their details. If not, check ‘No.’
11. Declaration


Confirm info is true, note the application date, and sign. If you’re under 17, a parent/guardian signs.
